Overview

The Pharmacy Department at the Dana-Farber Cancer Institute places emphasis on the values of reliability, professionalism, cooperation, teamwork, communication, positivity and inclusiveness.   The Medication Reconciliation Pharmacy Technician Supervisor is responsible for overseeing the medication reconciliation process within the pharmacy department. This role involves supervising a team of pharmacy technicians, ensuring accurate medication histories are obtained, verifying medication lists, and coordinating with healthcare providers to optimize patient medication regimens. The supervisor will also be involved in developing and implementing protocols to improve medication reconciliation safety and efficiency. Supervisor will staff in pharmacy locations/areas as required.

Responsibilities

Supervisory Duties:

  • Supervise and mentor a team of medication reconciliation pharmacy technicians, including training, scheduling, and evaluations.
  • Provide guidance on complex medication reconciliation cases and resolve any issues that arise.
  • Foster a collaborative environment within the pharmacy team and with other healthcare professionals.
  • Oversee the process of obtaining and verifying accurate medication histories from patients, caregivers, and other sources.
  • Ensure that medication lists are complete, accurate, and up-to-date.
  • Collaborate with healthcare providers to resolve discrepancies and make necessary adjustments to medication regimens.
  • Develop and implement policies and procedures to enhance the medication reconciliation process.
  • Monitor and evaluate the effectiveness of medication reconciliation practices and make recommendations for improvement.
  • Conduct regular audits to ensure compliance with pharmacy standards and regulations.
  • Ensure that patients receive accurate information regarding their medications and potential interactions.
  • Maintain and update documentation related to medication reconciliation activities.
  • Prepare reports on medication reconciliation metrics and outcomes for pharmacy leadership.

Medication Reconciliation Pharmacy Technician Duties:

  • Practices as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards.
  • Conduct in-person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list.
  • Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.
  • Communicate drug-drug interactions or other medication changes with pharmacists and providers to maintain patient safety. Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records.
  • Provide cross coverage in the outpatient/specialty pharmacy as needed to maintain seamless patient care.
  • Utilizes multiple systems such as EHR (Electronic Health Record), Inventory Management Systems and other systems to complete and document duties. Troubleshoot as necessary. 
  • Trains other employees as assigned.
  • Communicates and escalates issues or problems.
  • Performs other duties as assigned.

SUPERVISORY RESPONSIBILITIES:

  • Yes, supervises medication reconciliation technicians.  Supervisory responsibilities include but are not limited to participation in scheduling, orienting and training staff, performance management, and process improvement.

Qualifications

  • High School graduate or currently enrolled in an accredited entry-level degree program for Pharmacy or a related field.
  • 8 years of prior work as a pharmacy technician in a retail or hospital environment required. 
  • 4 years of experience with patient care or oncology medications required. 
  • 1-2 years in a supervisory or leadership role preferred.
  • Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required. 
  • National Pharmacy Technician Certification (PTCB) must be obtained within 1 years of hire date. 

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Ability to accurately perform pharmaceutical calculations.
  • Ability to utilize pharmacy technology systems.
  • Ability to develop proficiency and mastery of compounding of non-sterile products.
  • Ability to independently troubleshoot issues and assist in resolution or escalation to the appropriate resource.
  • Ability to treat all others (fellow pharmacy staff, patients, nurses, providers, support personnel, etc.) with the utmost professionalism and respect.
  • Ability to effectively work within a collaborative team-focused environment.
  • Participate in department meetings and functions to promote a successful and efficient pharmacy workgroup.
  • Ability to effectively communicate both orally and in writing.
  • Demonstrated critical thinking skills (ability to analyze information objectively and make a reasoned judgment).
  • Strong attention to detail.
